Triad Health Project offers free case management services for people living with HIV in Guilford County. Our support is rooted in compassion, dignity, and real connection. No matter where you are in your journey, our dedicated team is here to help you navigate the medical system. Together, we can make your care feel accessible, manageable, and human.

We offer three levels of case management services:

Medical Case Management

For people needing ongoing support accessing treatment and care, this program connects you with a dedicated case manager. Your case manager will work alongside you to navigate challenges, stay in medical care, and support your health goals.

Your case manager will regularly check in with you, help address barriers to taking medications or attending appointments, and assist with applications for medication assistance programs like HMAP. You may also receive bus passes for medical visits, access to our client food pantry, and emergency financial assistance when available.

Ryan White Support Services

If you are virally suppressed and regularly engaged in care, you may not need ongoing case management. You can still access important support services through THP.

This program includes help applying for medication assistance programs like HMAP, access to our food pantry, bus passes for medical appointments, and eligibility for emergency financial assistance. We’re here when you need us.

Triad Health Project Assistance Program (TAP)

For individuals who are virally suppressed and have lower support needs, TAP offers continued connection to essential resources in a simple, accessible way.

Participants can access the THP food pantry and receive bus passes for appointments or other essential travel needs. While TAP does not include case management or financial assistance, it helps ensure community support remains within reach.

All THP services are provided free of charge through funding from the Ryan White Act.
