Triad Health Project offers free case management services to individuals living with HIV in Guilford County. No matter the level of service, our dedicated team is here to make your journey through the medical system a little easier.
Care you can count on.
We offer three different levels of case management services:
Medical Case Management
Your case manager will contact you regularly to make sure there are no barriers to taking your medicines and going to your doctor. You will also receive support in applying for medication assistance programs, such as HMAP. You are eligible to receive bus passes for doctors appointments and access to our client food pantry. In the event of an emergency, you may be able to receive financial assistance.
Ryan White Support Services
If you are virally suppressed and in regular care you might not need a regular check-in from your case manager. You will still be able to receive bus passes, food from our pantry, and are eligible for emergency financial assistance. You will also receive support in applying for medication assistance programs, such as HMAP.
Triad Health Project Assistance Program (TAP)
For individuals that are virally suppressed and very low need, this program allows you access to the THP food pantry, and bus passes, but does not offer case management services or financial assistance.
All of our services are completely free with funding from the Ryan White Act.
